Position Overview:
The Care Management Program Coordinator works under the direction with the Director of Care Management to manage the daily operations of the care management program including supervising a team of health home care managers.
Responsibilities include supporting completion of documentation for staff and coordination of services, and coaching staff in the provision of person-centered services, responsibility for quality assurance and corporate compliance activities, data management, carries a caseload and other projects as assigned by the Director of Care Management.
Qualifications:
Bachelor's Degree; minimum three-year behavioral health experience and care management experience.
At least two years of supervision experienceMust have excellent computer/EHR, communication, interpersonal, organizational, and administrative skills.
Ability to multi-task, be able to work independently, and be willing to work both in office and in the field.
Driver's license with satisfactory driving record and own car for fieldwork required.
NYS Justice Center Criminal History background Check clearance through fingerprinting search required, in addition to clearance through the Protection of People with Special Needs Staff Exclusion List, the NYS Medicaid Exclusion List and clearance by the NYS Office of Children and Family Services for instances of child abuse and/or neglect.
